PLEASE PROVIDE THE FIVE MENU ITEMS THAT YOU WILL BE SERVING DURING THE MIDWEST STREET FOOD XPO.
YOU MAY SERVE MORE BUT THESE FIVE DISHES WILL BE ADVERTISED TO THE PUBLIC.
Certain fields above were not required at this time but will need to be submitted prior to the event date. Those fields include a copy of your Insurance Coverage Policy, your Health Department Certificate and your menu items. If you did not include them during this application process you will receive a subsequent email from us if accepted as a Vendor and you will be required to submit those documents following the cutoff deadline date and prior to the event date. Failure to submit these documents will void your space reservation and no refund will be provided to approved applicants.
